EGBN Reporting

As a member of United Way of Westchester and Putnam’s Essential Goods for Basic Needs (EGBN) Program, you are responsible for submitting reports on any donations your organization received. If no donations were received or all donations have been reported on, you do not need to complete a report. We understand that your organization may not have the approximate numbers available, so we ask that you answer to the best of your ability and provide estimated values when necessary.

You are required to submit a report starting the quarter after a donation was received by your organization and until the donation is fully distributed.

If you distribute donations at an event (e.g. pantry distributions, community events, etc.), you can report the information on the event (e.g. 50 boxes distributed throughout the day), rather than each individual that receives donations at the event. Outside of events, you will need to report information for each client that receives donations (e.g. Client A received 3 boxes of donations).

 

Step 1: Download the relevant report and fill it in.

Step 2: Save the report to your computer. Upload the saved report using the “Choose File” buttons in the form below.
